If you have chosen a publishing program
that includes consultation services, please complete this form to schedule your consultation. Cereb Press must receive
this form at least 24 hours in advance of your requested day and time. This allows Cereb Press to set aside the adequate
time to serve your needs effectively.
If you have already used the free consultation time included in your publishing
program package, you will be billed in half-hour increments (17.50 per half-hour / half-hour minimum) for one-on-one phone
consultations. You will receive an electronic bill at the end of each month, which will be due upon receipt. Please provide
as much advance information as possible so we can get right to the point and not waste your valuable minutes.
You
may also request an e-mail response to your consultation subject matter. E-mail consultation responses are always
free and you can request e-mail assistance as often as needed. If you prefer an e-mail response instead of a phone response,
